VBS VI: Medical Retina

Session 3 began with Dr. Netan Choudhry discussing the techniques of capturing OCT images of peripheral pathology. He showed several methods of obtaining peripheral scans including internal and external fixation methods, increasing the OCT B-scan length, using a 20 diopter trial frame, and OCT montage. He showed some fantastic OCT images of an astrocytic hamartoma, a VHL lesion, and an ora serrata pearl.

Next Dr. Jay Duker imparted 10 great pearls for the management of retinal vascular diseases. These included:

  • Hard exudates may get worse then round up before the patient gets better
  • Asymmetric diabetic retinopathy is not associated with ipsilateral carotid disease
  • Retinal cysts over scars in wet AMD are usually not VEGF mediated
  • Late diabetic vitreous hemorrhage is a tractional disease; consider taking the patient with full PRP and vitreous hemorrhage to the OR for a vitrectomy as the bleed is likely not VEGF mediated

Finally, Dr. Charlie Wykoff showed multiple cases to the panel, sparking a vigorous debate on when to take a traumatic pediatric macular hole to surgery. Polling the audience revealed about 10% of the room was using telemedicine for follow up or detection of wet AMD. 20-30% of the room had done a vitrectomy for refractory DME within the past month, and 1/3 of the room was using genetic testing in inherited retinal diseases.
